Steps for GUIDE program
enrollment

Step 1: Identify

Eligible patients with an existing or suspected dementia diagnosis.

Step 2: Refer

Direct patients to Synapticure through the referral form or by calling.

Step 3: Outreach

Synapticure contacts patients and caregivers for assessment.

Step 4: Diagnosis

Synapticure practitioner confirms a diagnosis of dementia.

Step 5: Enrollment

Synapticure submits patient for GUIDE enrollment to CMS.

Step 6: Outcome

Accepted: Care through the GUIDE program.
Not Accepted: Offer other neurological support.

Step 7: Communication

Synapticure communicates results of assessment to PCP.

What is GUIDE?

Guiding an Improved Dementia Experience (GUIDE) is a new Medicare program that focuses on improving the quality of life of patients living with dementia and their caregivers at no additional cost. Beginning July 1st, this 8-year initiative focuses on dementia care management to reduce strain on unpaid caregivers and enable people living with dementia to safely remain in their homes and communities.

Approximately 10% of Medicare beneficiaries have dementia and may be eligible for GUIDE. This program presents a unique opportunity to enhance patient health outcomes, alleviate financial strain, and bridge gaps in access to care, particularly in underserved areas, ensuring equitable support for all individuals affected by dementia.

What benefits does GUIDE provide to patients?

Although GUIDE offers additional services for patients and caregivers, the program doesn’t affect your relationship with your patients—you’re still their primary provider.

Through Synapticure, ACOs and PCP practices receive access to:

  • Specialized neurologists and psychiatrists within 1 week of referral
  • End-to-end care coordination, from transitions of care management to connections with local resources
  • Faster diagnosis from onset of symptoms, which leads to better outcomes
  • interdisciplinary team to provide wraparound care and frequent touch-base with care dyad to facilitate respite care and other services
  • Support with transition of care
How can my patients access the program?

Synapticure, partners with PCPs, ACOs, and health systems to provide Medicare Beneficiaries access to the program. Partnering with Synapticure does not impact attribution or your ability to bill for services (e.g. CCM). You remain the primary provider for patients in your network.

  • Eligibility requirements: Patients enrolled in Traditional Medicare (not Medicare Advantage) with a suspected or confirmed dementia diagnosis, who are living at home or in the community and are not participating in hospice or a PACE organization are eligible.
